Your first trip to Burrell is pretty simple...

You'll meet staff who are friendly and easy to talk with. After you complete some general paperwork you'll meet someone who will ask you a lot of questions.

Your answers to those questions will help them provide the best options for your situation.

Here's what to need for your appointment:

  • Driver's license
  • Insurance card
  • Social Security number
  • If you're a minor, bring a guardian
