Case Management
Residential Care Facility
This program provides community support for clients residing in a Residential Care Facility within Burrell’s catchment area. Community Support Specialists assist clients by helping them learn to manage their illness and behavior in a way that will increase their skills and foster the highest level of independence possible. They also assist clients in accessing transportation, employment, therapy, medical treatment, substance use disorder treatment/support, etc., and coordinate necessary assistance from Department of Mental Health-Supported Community Living. Burrell's team will monitor and work directly with the facility to ensure that the residents are in a safe living environment and their needs are met.
Cost Details
Our services at Residential Care Facilities are funded one of two ways. It can be funded through an individual’s SSI/SSDI, or through the Department of Mental Health funds that are available for those who meets criteria. Individuals must have Medicaid coverage for case management services.
