OUR SERVICES

Case Management

In some cases a clients diagnosis may mean they need additional support to function at home, work or in the community. We understand the challenges that come into play for many of our clients so we provide support in their homes and in the community. The goal of case management program is to support and equip clients with successful independent living skills. Clients must meet the criteria established for each program to participate in services.

Social Worker

Assertive Community Treatment (ACT)

ACT - Assertive Community Treatment uses a multidisciplinary team to assist wherever needed to accommodate the acute service needs of clients.

Traditional Age Youth

Assertive Community Therapy - Transitional Aged Youth (ACT-TAY)

ACT-TAY-Assertive Community Treatment for Transitional Aged Youth, uses a multidisciplinary team to assist wherever needed to accommodate the acute service needs of clients.

Youth Community Support Services

This community program is available for families in need of support for emotional and behavioral concerns with their children.

HIV/AIDS Support Services

This program assists individuals to improve their quality of life by helping them maintain their healthcare and support systems, as well as coordinating resources and advocating for each individual’s needs.

Adult Community Psychiatric Rehabilitation

Adult Community Psychiatric Rehabilitation (CPR) services focus on the complex needs of individuals with serious and persistent mental illnesses. We provide specialized services to address mental health needs like poverty, connections to local treatment providers, and resources to stabilize the individual.

Homeless

Homeless Services

Homeless services supports the delivery of outreach and services to individuals with serious mental illnesses and those with co-occurring substance use disorders who are experiencing homelessness or at imminent risk of homelessness.

Employment

Individual Placement Support Employment Program (IPS)

IPS is a time unlimited individualized support that utilizes a team approach. Our staff work alongside mental health treatment, vocational rehabilitation and community based employers to assist people with finding competitive employment as a way to enhance recovery.

Multi-generation

Psychosocial Rehabilitation (PSR)

This program is for adults enrolled in CPR who require structured social activities and community connectedness to maintain their mental and emotional wellbeing.

Integrated Treatment for Co-Occurring Disorders

This integrated program serves those with a severe and persistent mental illness as well as a co-occuring substance use disorder.

Case Management for Residential Care Facilities

Community Support Specialists assist clients in accessing transportation, employment, therapy, medical treatment, substance use disorder treatment/support, etc., and coordinate necessary assistance from Department of Mental Health-Supported Community Living.

Healthcare Home

Healthcare Home

Using a team approach, Burrell staff, clients, families and primary-care providers coordinate comprehensive primary physical care to Medicaid patients with behavioral health and/or chronic physical health conditions in a way that recognizes all of their needs as persons, not just clients.
