What to expect at your first appointment

Burrell offers an array of services and it can be tricky to figure out which ones will he helpful for your situation. We have set up an evaluation process for all new clients that will help guide you to the appropriate care. Below is a list of items to bring with you to the appointment so that we can get everything we need to get you in the system and make your first meeting with us impactful.

Our physical locations and Connection Centers remain open with increased safeguards for your safety and that of our staff, but we ask you to call to to decide whether telehealth services are a good fit for your needs.

What to have handy at your appointment

Driver's License, Insurance Card, Social Security Number

If you are a minor, you will need to bring your legal guardian. Please bring any paperwork about guardianship if your guardian is not your biological mother or father.

There will be some general paperwork to complete before meeting with your evaluator. During your evaluation meeting it will be determined which Burrell services will fit best for your treatment needs.
